About the Program
Home-based Complementary Diabetes and Hypertension: Developing a scheme which will help
in follow up and effective management of diabetes and hypertension patients
even out of hospital. This is a method of supporting patients but also
providing population based NCD prevention services via the home.
Comunity Center
-Develop an effective tool to be used for the follow up.
Data Driven
-Provide NCDs prevention and control education to other house hold members.
Focus Support
-Reduce the burden of NCDs in the communities.
Why we do this.
This program came about because many NCDs patients lack follow up and this has been one of the reasons why NCDs morbidity and mortality has been on the rise. RADA therefore hopes to use this approach to not only follow up but also meet a wider population with NCDs education.
