Hypertension amongst diabetic patients is a worldwide public health challenge and a leading modifiable risk factor for other cardiovascular diseases and death. Also, compared with other cardiovascular disorders, hypertension is the most common comorbid disease in diabetic patients. According to the

Hypertension amongst diabetic patients is a worldwide public health challenge and a leading modifiable risk factor for other cardiovascular diseases and death. Also, compared with other cardiovascular disorders, hypertension is the most common comorbid disease in diabetic patients. According to the 2009 WHO report, up to 80% of people with diabetes will die of cardiovascular disease, especially hypertension and stroke, since most patients with diabetes develop hypertension.

This is a pilot community, healthcare and population platforms-based intervention for D & H in Cameroon where community health workers, technology, home based follow up collaboration among care and public health experts, and advocacy come together for improved health outcomes for people living with hypertension and diabetes and their families and by extension, the society.

THREE MAIN OBJECTIVES FOR THE PROGRAM

  1. Raise awareness for early diagnosis of diabetes and hypertension 
  2. Assist in the complementary follow-up of diabetes and hypertension patient
  3. Provide support for the effective management of diabetes and hypertension at home. 

THE PURPOSE OF THIS PROGRAM

To create a platform where community health workers, technology, home-based follow-up collaboration among care and public health experts, and advocacy come together for improved health outcomes for people living with hypertension and diabetes and their families, and by extension, the community.

Raise awareness

The  Program raises awareness on diabetes and hypertension by promoting prevention, ans early detection.

Assist in the complementary follow-up of individuals living with diabetes and hypertension to support effective management and continuity of care

Provide support

Provide support to individuals living with diabetes and hypertension to improve care, adherence, and overall well-being
